Each year, many children who want to participate in school or community sports are sidelined because they don’t have health insurance. They may not be able to afford a physical or their families worry they’ll get hurt on the field. Without health insurance, they can’t play.
CHIP and Medicaid are available now! This summer, the Centers for Medicare & Medicaid Services (CMS) is launching Get Covered. Get in the Game. in seven pilot states, including Colorado, Florida, Maryland, New York, Ohio, Oregon, and Wisconsin. The pilot is designed to bring coaches, schools, families and communities together to raise awareness about available health coverage and get eligible children enrolled. Getting covered will protect them both on and off the field.
Nearly five million uninsured kids are eligible for health coverage today. Get Covered. Get in the Game. supports the Connecting Kids to Coverage challenge issued by HHS Secretary Kathleen Sebelius in February, which calls on government and community leaders, the faith community, and others to help enroll the nearly five million kids who are eligible for CHIP or Medicaid but remain uninsured.
CMS has developed the materials below to provide information to families and to train coaches about CHIP and Medicaid, so they can help promote the programs throughout the entire school community.

The 2010-2011 School Year is right around the corner and that means Fall sports and activities are as well. We encourage each and every student to participate in some form of extracurricular offering during the school year. It is proven that involved students are better students!
Below under athletic forms you will find the paperwork and information that you will need to begin practicing on the first day of your activity. Most coaches will be sending out additional information in the mail shortly, if you have not already received a mailing.
The MANDATORY PARENT/ATHLETE meeting for high school students, grades 9-12, will be held on Tuesday, August 10th at 6:00 pm in the High School Gym. Please print-off the forms that you will need, fill them out, and bring them with you to the meeting with your coaches. Activity fees are also payable on this date and checks for $25 can be made payable to SCF Schools.
The number of athletes doing summer preparation has been totally impressive and I am very proud of our young Saints as they are preparing every day for the upcoming 2010-2011 seasons. With preparation comes success and I am really looking forward to this year of exciting events and many successes by our student/athletes. If you have not been participating in summer weightlifting and conditioning opportunities, please - it is not to late to start! Weightroom hours are daily until August 6th:
Morning Session 6:30 – 7:30 am Mon-Fri
7:30 – 8:30 am Mon-Fri
8:30 – 9:30 am Mon-Fri
Evening Session 6:30 – 7:30 pm Mon-Thurs
Adults and out-of-district residents are welcome to participate.
Open weight room is free for district students. Adults and out-of-district residents must pay a $1 per time usage fee.
The fall schedules are conveniently located on the home page of the website as they become confirmed and also on each individual sport page. These are also printable for your convenience.

A friendly reminder:
Summertime is defined as the last day of school to the first day of school.
There are some leagues around the state getting ready to start summer practice and play. I would advise organizers, especially in pre-season and summer leagues to see that teams are as diverse as they can possibly be during the school year which has not ended for some of our member schools. No team should be made up exclusively of students from the same school. Merely grade level diversification is not acceptable.
The rule applies outside the sport season during the school year and not during the summer. If you look at the team, does it look like a school team?
Parents and athletes should be reminded that participation in a non-school competition or run in the same sport during the season is not allowed by member school WIAA rules.
Summer Contact rules apply from the last day of school until the first day of school. Coaches and schools may have five unrestricted days of contact with their athletes before July 31. Unlimited "non-school" contact is allowed in the sports of baseball, cross country, golf, gymnastics, softball, swimming & diving, tennis, track & field and wrestling.

Don’t Wait Until the
Last Minute!
PHYSICAL EXAM CARD * (green): Anyone who desires to participate in the Wisconsin Interscholastic Athletic Program (WIAA) must have their physical exam information card on file in the Activities Office prior to the first day of practice. A licensed physician must sign the appropriate form.
ALTERNATE YEAR CARD * (buff): Must be completed on the alternate year. Alternate year is the second year of a physical taken after April 1st.
Cards are available in the Activities Office or at your clinic.
If your child needs a physical and your family cannot afford it, please contact the office so we can help you fund a physical 715-483-2507 ext. 1406.

- Your athletic physical or parent permit card.
- Your Student Information Card that was mailed to you prior to this meeting, with corrections.
- Your activity fee ($15) made payable to SCF Schools. (Per activity)
- Your High School Athletic Eligibility Information Bulletin; filled out and signed by parent and athlete.
- Your signed Sportsmanship Pledge Form, signed by parents and athlete.
- Your signed Activity Code, signed by parents and athlete.

The WIAA is against the use of anabolic-androgenic steroids and other performance enhancing substances (PES). Drugs that are illegal or could be harmful if taken in excess amount:

HEADS UP - PLAY IT SAFE
Each year, more than seven million high school students participate in interscholastic sports in the United States. For instance, there are 1.2 million high school football athletes, compared to 38,000 college and 1,700 professional football players. There are approximately 1.4 million sports injuries, according to a 2006 national sports injury survey.
The good news is that injury rates have dropped by half in the last decade as sports medicine techniques have improved, along with a growing awareness of the importance of preventing and treating injuries, said Dawn Comstock, an assistant professor at the Ohio State University College of Medicine. She administers the survey, which tracks sports-related injuries at 100 high schools throughout the country.
Coaches are instructed on how to spot a concussion, but having parents know the same information is important, as the kids go home with you.
The signs of concussion are not always well recognized. And because of that, teens may put themselves at risk for another injury. For example, players may return to a game before they should, thinking nothing's wrong. That's a problem, because if the brain hasn't healed properly from a concussion and someone gets another brain injury (even if it's with less force), it can be serious.
Repeated injury to the brain can lead to swelling, and sometimes people develop long-term disabilities, or even die, as a result of serious head injuries. So it's really important to recognize and understand the signals of a concussion.
Although we may think of a concussion as someone losing consciousness (passing out), a person can have a concussion and never lose consciousness.
Symptoms of a concussion may include:
- "seeing stars" and feeling dazed, dizzy, or lightheaded
- memory loss, such as trouble remembering things that happened right before and after the injury
- nausea or vomiting
- headaches
- blurred vision and sensitivity to light
- slurred speech or saying things that don't make sense
- difficulty concentrating, thinking, or making decisions
- difficulty with coordination or balance (such as being unable to catch a ball or other easy tasks)
- feeling anxious or irritable for no apparent reason
- feeling overly tired

MISSION/PHILOSOPHY
The School District of St. Croix Falls believes that one of the primary purposes of education is to provide each student with the values and training necessary to lead a rewarding and happy life. The St. Croix Falls District is pleased to have you participate in its co-curricular program. As an integral part of the total school experience, the program is designed to help students become successful individuals and better citizens of our school, community, state, and nation. Additionally, the activities and sports programs allow each participant to make a personal commitment to healthy involvement in activities, which expand and enrich their academic program. These activities provide opportunities for the youth of this community to develop the proper ideals of sportsmanship, ethical conduct, and fair play. Participation in this school program is a privilege for which every student should be willing to meet the challenges deemed necessary to fulfill these ideals. It is with this in mind that the School District of St. Croix Falls Activity Code has been established. The intent of this co-curricular policy is to provide you and your parents or guardian with a reference to your responsibilities and the school's expectations for your participation in the St. Croix Falls co-curricular program. No code can be all-inclusive. Whenever conduct or behavior is determined to be inappropriate a consequence will be appraised.
OBJECTIVES
1. To provide the best possible instruction to learn the skills of competitive and lifetime activities.
2. To develop within the students the determination and dedication necessary to complete their chosen goals.
3. To develop the self-discipline, self-confidence, and self-understanding necessary for a successful life.
4. To develop a willingness to place the good of the group before desires of the individual.
5. To teach that a consequence follows the violation of a rule.
6. To provide a "whole school" interest and activity that will develop school pride in all students.
7. To provide opportunities to exemplify and observe good sportsmanship.
8. To provide the opportunities for developing lasting friendships, both with teammates and opponents.
9. To provide a foundation for building good school moral and community involvement.
Athletic Fees
All students in grades 6-8 participating in extra-curricular activities must pay a $20.00 fee per activity. All students in grades 9-12 pay $25 per activity. Family cap at $100 Fees must be paid to the coach prior to the first practice.
Admission Prices
Adults: $3.00 Students : $1.00 (Tournament prices may vary)
Athletic Passes are also available whenever tickets are being sold or in the High School Office. The cost is $60 per family, $30 per adult, $20 per student (1st grade - college senior)
Senior Citizens: All residents of the St. Croix Falls School District over the age of 55 are eligible for a Senior Pass good for all home athletic events, with the exception of tournaments. These passes are free of charge and available at the St. Croix Falls High School Office.

Student Athlete Information
www.ncaaclearinghouse.net is the NCAA Guide for the college bound student-athlete. To register, the prospective student-athlete should access the registration materials by visiting the clearinghouse web site. From the home page, the prospect should click on "Prospective Student-Athletes", which will link the student-athlete to the necessary information. It is strongly suggested that prospective student-athletes who plan to participate in intercollegiate athletics at an NCAA Division I or II institution use this on-line registration. It is extremely important that prospective student-athletes read the guide carefully and understand the NCAA initial eligibility requirements.
